Human Error Perspectives in Aviation
As aircraft have become more reliable, humans have played a progressively more important causal role in aviation accidents, resulting in the proliferation of human error frameworks and accident investigation schemes. To date, however, few efforts have been made to systematically organize these different approaches based on underlying theoretical similarities, and formalized methods for evaluati...
متن کاملOn the Over-Emphasis of Human ‘Error’ As A Cause of Aviation Accidents: ‘Systemic Failures’ and ‘Human Error’ in US NTSB and Canadian TSB Aviation Reports 1996-2003
It has been claimed that up to 80% of all aviation accidents are attributed to human ‘error’ (Johnson, 2003). This has important consequences as national and international initiatives focus on the reduction of operator ‘error’, for instance by increasing the levels of automation in Air Traffic Management. However, it is difficult to validate claims about the frequency of human ‘error’. This pap...

متن کاملHuman Error and Commercial Aviation Accidents: An Analysis Using the Human Factors Analysis and Classification System
OBJECTIVE The aim of this study was to extend previous examinations of aviation accidents to include specific aircrew, environmental, supervisory, and organizational factors associated with two types of commercial aviation (air carrier and commuter/ on-demand) accidents using the Human Factors Analysis and Classification System (HFACS). BACKGROUND HFACS is a theoretically based tool for inves...
متن کاملHuman-centred design in aviation
This paper focuses on the next challenges that, in the near future, ergonomics has to cope with in the aviation domain. After a short excursus, showing the accidents dynamics along the years and pointing out the relative causes, the paper illustrates the difference between two different conception of automation: a generic human (user) friendly versus a specific pilot-friendly concept. This is u...
